Dr. Crites was born and raised in Indiana. He completed his residency at Duke University. A fellowship in Adult Reconstructive Surgery was completed at the Center for Hip and Knee Surgery in Indiana. A sports medicine fellowship at the University of Maryland completed his orthopaedic training. During his fellowships, he provided team coverage for the Baltimore Ravens, Duke University, and the University of Maryland. After his sports medicine fellowship he joined the staff at the University of Maryland for three years. During that time he was also the team doctor for the Baltimore Ravens.

Dr. Crites is board certified in Sports Medicine and is a team physician for the Iowa Energy, the Iowa Barnstormers and Graceland University. He is also the team physician for both Ankeny High School football teams. He is an avid fisherman, hunter, master griller, and musician.

Dr. Formanek received his medical degree from the University of Iowa School of Medicine. His internship was at the University of Nebraska Medical Center. He completed a 5 year residency in Orthopaedic Surgery at the Combined Orthopaedic Residency Program of Creighton University / University of Nebraska Medical Centers. He moved from Omaha, NE to Boston, MA for his Hand and Upper Extremity Surgery Fellowship at Harvard University. His experience included training at Brigham and Women’s Hospital and Children’s Hospital on the Harvard Medical School campus.
Dr. Formanek became Board Certified by the American Board of Orthopaedic Surgery on July 15, 1994. He has been recertified in Orthopaedic Surgery by the Board in 2004 and 2014. Dr. Formanek became Board Certified in Hand Surgery by the American Board of Orthopaedic Surgery on August 28, 1995 and has been recertified by the Board in 2004 and 2014. Dr. Formanek is the only Board Certified Hand Surgeon at Capital Orthopaedics and Sports Medicine.
Dr. Formanek has been a Member of the American Medical Association since beginning practice in 1992. He has been a Member of the American Society for Surgery of the Hand since 1995. Membership to the Society is only available for Board Certified Hand Surgeons. He is also a member of the Iowa Orthopaedic Society, Iowa Medical Society, and Polk County Medical Society.
Dr. Formanek has privileges at all Unity Point Hospitals and Surgery Centers in Des Moines, Iowa. Dr. Formanek has privileges at all Mercy Medical Center Hospitals and Surgery Centers in Des Moines, Iowa. He also has privileges at West Lakes Surgery Center and Surgery Center of Des Moines. He is the Medical Director of West Lakes Surgery Center.
Dr. Jill Frerichs is a native of Hinton, Iowa. She attended the University of Iowa & subsequently earned her Doctor of Podiatric Medicine (DPM) degree at Des Moines University in 2001. Dr. Frerichs completed a three year foot and ankle surgical residency at Grant Medical Center in Columbus, Ohio.
Prior to joining Capital Orthopaedics in September 2011, Dr. Frerichs served as an assistant professor of podiatric surgery at Des Moines University for five years. She is board certified in both foot surgery and reconstructive rear foot and ankle surgery by the American Board of Podiatric Surgery.
Dr. Frerichs specializes in foot and ankle reconstruction, ankle arthroscopy and foot and ankle injuries. Dr. Frerichs has office locations in Clive, Ankeny, Knoxville, Pella, Mt. Ayr and Leon. She enjoys spending time with her husband and two children, participating in outdoor activities and running.
Dr. Fish is a native of Des Moines and graduated in 2001 from Des Moines University and completed training in orthopaedic surgery in Michigan. He continued his training in Indianapolis with a Fellowship in Joint Replacement and Sports Medicine. He is a member of the clinical faculty and a lecturer at Des Moines University. Dr. Fish is the former President of Adult Reconstruction Arthritis Section, for the American Osteopathic Academy of Orthopaedics. He is also a fellow of the American Osteopathic Academy of Orthopedics

Dr. Fish returned to Des Moines with his wife and six children and joined Capital Orthopaedic and Sports Medicine in January 2010. Dr. Fish brings his expertise with shoulder, hip and knee reconstructive surgery and arthroscopy to the practice. He is an experienced pilot and guitar player. He also enjoys skiing, water sports, and spending time with family.
