Dr. William Boulden - MD, FAOSS
Dr. Boulden was raised in Southwest Iowa.
After graduation from high school in Lennox, Iowa, Doctor Boulden attended the University of Iowa and graduated with a B.S. in pharmacy. He then attended and graduated from the University of Iowa Medical School. Doctor Boulden spent 2 years in general surgery residency at the medical college of Wisconsin in Milwaukee. He completed an Orthopaedic Residency Program at the University of Nebraska in Omaha.
Since November of 1977, Doctor Boulden has practiced orthopaedic surgery in Des Moines, Iowa. Doctor Boulden was Board Certified in orthopaedics in 1979. He became a Fellow of the American Academy of Orthopaedic Surgeons in 1982.
Doctor Boulden is a member of the American Medical Association, Iowa Orthopaedic Society, and Fellow of the North American Spine Society.
Doctor Boulden is a general orthopaedic surgeon with special interest in lumbar spine surgery, total knee replacement surgery, and total hip replacement surgery.
Doctor Boulden is on staff at Mercy Hospital Medical Center, Iowa Lutheran Hospital, Des Moines General Hospital and Iowa Methodist Medical Center. He has held several appointed and elected positions in these hospitals, including President-Elect at Iowa Lutheran Hospital, Chief of Orthopaedics at Iowa Lutheran Hospital, Vice Chairman of Orthopaedics at Mercy Hospital Medical Center, and Vice Chairman of surgery at Iowa Lutheran Hospital.
Doctor Boulden believes in practicing cost-conscious, quality-care medicine. He gives his patients plenty of time to discuss their medical problem and proposed treatment. Doctor Boulden makes certain that all of his patients' questions are answered. He wants his patients to understand all aspects of their medical problem and proposed treatment so that his patients can make informed decisions.
Doctor Boulden's special interests include spending time with his family and participating in outdoor activities.
Dr. Jacobson is board certified orthopaedic surgeon. He received his doctor of medicine degree from the University if Iowa in Iowa City. Following medical school, Dr. Jacobson completed his internship and residency at the University of Pittsburgh Medical Center in Pittsburgh, PA.
He completed a Sports Medicine fellowship at the University of Minneapolis Sports Medicine Center in Minneapolis, MN. During the fellowship, he worked with team physicians for the Minnesota Vikings, Timberwolves, and the Twins.
Dr. Jacobson is a member of the American Academy of Orthopaedic Surgeons, American College of Sports Medicine, American Medical Association, Arthroscropy Association of North America, Iowa Orthopaedic Society, and Iowa Medical Society.
Dr. Jacobson prefers joint replacement surgery for knees and shoulders.
Dr. Michael Lee was born and raised in Clarinda, Iowa. He attended the University of Northern Iowa where he majored in Biology. Dr. Lee earned his Doctor of Podiatric Medicine (DPM) degree at Des Moines University. He then completed a three year foot and ankle surgical residency at the Western Pennsylvania Hospital in Pittsburgh, PA. His training in Pittsburgh focused on adult reconstruction of the foot and ankle, trauma, and sports injuries.
Dr. Lee has been in practice in central Iowa for 10 years. He is board certified in both foot surgery and reconstructive rearfoot and ankle surgery by the American Board of Podiatric Surgery. He has authored numerous papers and chapters regarding foot and ankle surgery. He also regularly speaks at regional, national, and international scientific meetings. Dr. Lee is currently president-elect of the American College of Foot and Ankle Surgeons (ACFAS), the scientific organization for over 6,000 podiatric foot and ankle surgeons in North America.
He specializes in reconstruction of the foot and ankle, ankle arthroscopy, and foot and ankle injuries/trauma. Dr. Lee currently has office locations in Clive (Westlakes Campus and Mercy West), Mercy North in Ankeny, Mercy South, and Iowa Falls. Dr. Lee enjoys spending time with his wife and two kids, golfing, and is a fervent Pittsburgh Steelers fan.
Dr. Formanek received his medical degree from University of Iowa. He Completed his Orthopaedic surgery residency at the Creighton-Nebraska Foundation in Omaha, Nebraska. Subsequently, he completed the Harvard Hand Fellowship at Brigham and Women's and Children's Hospital in Boston, MA.
He is certified by the American Board of Orthopaedic Surgery with a certificate of added qualifications in hand surgery.

Dr. Crites was born and raised in Indiana. He completed his residency at Duke University. A fellowship in Adult Reconstructive Surgery was completed at the Center for Hip and Knee Surgery in Indiana. A sports medicine fellowship at the University of Maryland completed his orthopaedic training. During his fellowships, he provided team coverage for the Baltimore Ravens, Duke University, and the University of Maryland.

Dr. Crites is board certified in Sports Medicine and is a team physician for the Iowa Energy, the Iowa Barnstormers, Graceland University and William Penn University. He is an avid fisherman, hunter, master griller, and musician. He and his family are excited to make Des Moines their new home.
Dr. Fish is a native of Des Moines and graduated in 2001 from Des Moines University. Dr. Fish completed training in orthopaedic surgery in Michigan. He continued his training in Indianapolis with a Fellowship in Joint Replacement and Sports Medicine. He is a member of the clinical faculty and a lecturer at Des Moines University. Dr. Fish returned to Des Moines with his wife and three children and joined Capital Orthopaedic and Sports Medicine in January 2010. Dr. Fish brings his expertise with shoulder, hip and knee reconstructive surgery and arthroscopy to the practice. He is an experienced pilot and guitar player. He also enjoys skiing, water sports, and spending time with family.
Dr. Yanish received his medical degree from St. George's University School of Medicine and then completed his general surgery residency at St. Joseph's Mercy Oakland Medical Center in Pontiac, MI. His Orthopaedic Hand Fellowship was completed at the University of Massachusetts in Worchester, MA. Dr. Yanish is Board Eligible in general surgery and has completed his written board certification examination.
