About the program
COSM's free sports physicals program offers middle and high school student-athletes a comprehensive pre-participation physical exam at no cost. Every athlete receives a primary care physical exam plus an orthopedic musculoskeletal screening from our specialists — the same team that serves as the official provider for the Iowa Wild. Athletes leave with a completed, signed IHSAA Pre-Participation Physical Evaluation form, accepted by all Iowa high schools for all IHSAA-sanctioned sports.

Need a copy of your form?
If your completed IHSAA form is missing, was lost, or you need a replacement copy, please reach out to us and we'll take care of you. Please include your athlete's full name, date of birth, school, and the event date you attended and we will follow up as quickly as possible.

What to Expect
More than just a check-the-box physical.
At Capital Orthopaedics & Sports Medicine, we do sports physicals differently. Every athlete receives a comprehensive exam that goes beyond the basics — so you can feel confident your child is ready to compete.
Your athlete’s visit includes:
- Primary Care Physical Exam — A head-to-toe evaluation covering heart, lungs, vision, and overall health. The same physical your pediatrician would do, performed by our sports medicine physicians who have specific primary care experience.
- Orthopedic & Musculoskeletal Screening — An additional evaluation by our orthopedic specialists, checking joints, flexibility, strength, and prior injury concerns. This is something most sports physical clinics don’t offer.
- IHSAA Clearance — Your completed and signed pre-participation physical form, ready to turn in to your school.
The whole visit typically takes about 30 minutes. You’ll leave with everything your athlete needs to be cleared to play.
